We recommend that you start by building skills that apply to both on-premise computing and cloud computing. If you already have on-premise skills, then you can enhance their skill sets to include cloud computing. Consult the table below for possible paths.
Recertification Requirements for Windows Azure Developers Windows Azure is a frequently updated cloud services operating system. The skills required by Windows Azure developers will evolve more rapidly than for non-cloud-based technologies. To ensure that our customers can have confidence that developers who hold the MCPD: Windows Azure Developer certification have kept pace with the evolution of Windows Azure skills, developers who hold this certification will be required to recertify every two years. Through recertification, we can maintain the value of the certification as cloud services mature over time. More information about this recertification requirement will be provided at a later date.

Great question.
You are correct. By taking either 521 or 523 to upgrade your MCPD on .NET 3.5, you earn the MCTS: Service Communications and MCTS: Data Access certifications that are requirements for earning the MCPD: Windows Azure Developer certification. Together with passing exam 70-583, you should be good to go.
